Caught a few more.
In Great Escape Hard, rooms 16703, 16503, and 16704 all have this bug. In those rooms, it's the 'standard' setup for the bug to appear, that is to say pulling a lever while Jack is standing on gold/silver blocks.
Now, here's the interesting bit. In room 19096 of Falcon Manor Easy (of all places!), there's a different variant of the bug. If Jack stands where he is in the screenshot (on the brick just above the lever), and Enter is pressed, Jack will auto-align with the lever, pull it, fall down 1 brick— and be unable to jump!
